What are the roles and responsibilities in your current position?
Provide support and advocacy to self-identified indigenous patient who are accessing services &/or been admitted to Brant Community Healthcare System.
To assist with coordination of discharge planning.
Providing access to community supports and resources.
Support with medical literacy.
Access to traditional supports and resources.
Support and advocacy through the patient relations process.
What barriers did you have to overcome to succeed?
Gaps with discharge planning and service coordination.
Solution: to ensure I have and maintain positive working relationships with community services and trust that the individuals will be supported in community to prevent re-admission when possible.
What is your advice for healthcare students?
Treat everyone with kindness, respect, dignity and honesty. When I am working with a patient, I provide the same level of care I would expect for my family; this makes a difference.
Be authentic in your interactions with other patients, families and colleagues.
